Swing Bed Program
The Swing Bed Program at Madison County Memorial Hospital is for patients
who no longer need the acute care environment, but still require special
care provided by the hospital. The Swing Bed Program is designed to prepare
the patient to return home or for placement in another facility that
provides skilled nursing care or other procedures and therapies
We are able to offer a convenient and more easily accessible alternative for
patients & families within the Madison County area. Our practice is focused on personalized
service, positive outcomes,
and is individualized to the patients needs, as well as the extent of
therapy indicated.
Our care team consists of physicians, nurses, physical therapists, speech therapists, respiratory
therapists, dieticians/nutritionists, activity coordinator, chaplains, and case
managers.
Services include skilled rehabilitation therapy, recreational
activities, religious services, social services, and discharge planning.
To Qualify
for Admission:
A patient must have had a hospital stay of three days or longer.
They may be admitted directly from the hospital or within 30 days immediately following a hospital discharge (for the same diagnosis)
The patient must have the need for Skilled Nursing Services, Physical, or Speech Therapies.

Madison County Memorial Hospital is a Medicare/Medicaid approved Critical Access Hospital that offers skilled nursing care and services such as physical therapy, speech therapy, radiology, laboratory, dietary services, case management, activities and patient/family education. Before discharge, patients are assessed to make sure they will be safe in the home environment and have all the equipment necessary for activities of daily living. We refer to medical equipment companies, home health agencies, and long-term care facilities in the Madison area to meet any needs the patient may have.
