Four Freedoms Health Services is owned and operated by Madison County Memorial Hospital.   It provides complete family-centered care and is designed to meet your family's needs

Four Freedoms Health Services is open Monday through Friday from 9 AM to 5 PM.  Walk-ins are accepted.

Our Health Team

sampleDr. Brett Perkins, M.D.

Dr. Perkins graduated from the University of Georgia at Athens, Georgia with a Bachelors Degree in Biology.  He attended medical school at the Medical College of Georgia at Augusta, Georgia earning his M.D. in 1994.  Dr Perkins continued his education in a family medicine residency program at Tallahassee Memorial Hospital in Tallahassee, Florida.  He is Board certified in Family Practice Medicine.  He and his wife, April have five children.

Ms. Gena Plain, P.A.

Ms. Plain graduated from Florida State University in Tallahassee with a Bachelors degree in Medical {Laboratory} Technology, and earned a masters degree in Physician Assistant Studies from the University of Florida College of Medicine in Gainesville, Florida in 2000.  Ms. Plain and her husband have two children.

Ms. Michele Richardson, ARNP, FNP-C

Ms. Richardson graduated from Valdosta State University with a Bachelors Degree in Nursing in 1990. She attended Florida State University and earned a Master Degree in Nursing Education and a Post Masters Degree as an Advanced Nurse Practitioner in 2008. Ms. Richardson and her husband have 4 children.

Dr. Perkins, Ms. Plain, and Ms. Richardson are members of the medical staff at Madison County Memorial Hospital.  Should admission to the hospital be necessary they can admit you and continue your care.
