Case Management
The purpose/mission of the Case Management program at Madison County Memorial Hospital is to be the patient advocate, empower patients and families by providing them with information to make informed healthcare choices, and provide information on important topics, such as Medicare benefits, Advance Directives, and Community Resources.The case manager coordinates the clinical team (being the patient, patient’s family/primary caregiver, physician, and ancillary medical staff) goals, and manages the financial and quality outcomes of the care provided.

The case manager assists the patient to move seamlessly through our continuum of care while promoting quality and achieving cost-effective outcomes that involve everyone.
Historically, case managers have been encouraged to focus on decreasing patient length of stay. At Madison County Memorial Hospital, our focus is to provide quality of care to the patient the extent of time necessary. This focus ranges from the quality of the physician documentation to post hospital continuum of care.

The case manager is actively involved with discharge planning to ensure patients receive appropriate care and are discharged to a safe environment. Madison County Memorial Hospital case management program anticipates the need for post hospital care and initiates discharge planning from the moment a patient is admitted to our facility.
